Subject to the Terms of Use HealthInfoNet (referred to herein as “HealthInfoNet,” “we,” “us” and “our”) provides you with access to this website, www.hinfonet.org (the “Site”).

1.  What information is collected on the Site?

Collection of Your Personal Information

We collect personally identifiable information (e.g., name, address, telephone number, e-mail address, date of birth, gender, social security number) that you knowingly provide through our Site.  For example, you may Opt Out or Opt In to participating in the HealthInfoNet System within the Site.  To do so, we request your full name, address, date of birth, gender, telephone number, e-mail address, and social security number (SSN is optional).  We use this information to respond to your Opt Out or Opt In request in accordance with this Privacy Policy.  Other pages on this Site may allow you to provide us with personal information to express interest in HealthInfoNet programs, ask a questions or request additional information.   

Information Sent to Us by Your Browser

Certain information about your computer hardware and software is automatically collected when you use the Site.  Such information may include: your IP address, browser type, domain names, pages within our Site that you visit, how long you stay on our Site and pages, the identity of your Internet Service Provider, the name and version of your operating system and referring sites. This information cannot be used to personally identify you unless you identify yourself to us.  We collect this information to learn what pages and content are most valuable to our users and to improve the Site.
 

Use of Cookies, Pixel Tags, Web Beacons and Similar Technologies

2.  How does HealthInfoNet use the information it collects?

In generally, we use the information collected on the Site to support and improve the operation of our Site and to provide users with the services they have requested.

With respect to personal information collected on the Site, HealthInfoNet uses such information to provide users with the services they have requested.  For example, Personal information entered into one of our secure forms, which includes our HealthInforNet System Opt-out form and Opt-in forms and our audit request form is used to process your request as set forth in the applicable form.

If you have provided us with personal information, we may use this to make you aware of new services HealthInfoNet is offering or to ask your opinion on surveys.

3.  Does HealthInfoNet share information with third parties?

We may share your information with trusted third party service providers to help us with management and operation of the Site, communications activities, web analysis, customer support and other important services. Although these third party service providers may not be affiliated with HealthInfoNet, they have agreed to maintain the confidentiality of your information and are not authorized to discuss, disclose or make any use of your personal information except as necessary to provide such services.

We do not sell, rent, or lease our user lists to third parties, however we may contact you on behalf of a trusted business partner of ours.

4.  What security measures does HealthInfoNet Use to safeguard personal information?

We adhere to industry standard practices and use industry standard technologies to secure your personal information from unauthorized access, use, or disclosure. We protect your personally identifiable information you provide us on private servers located in a secured and controlled, secure environment. Certain forms on the Site that collect personal information, such as our opt-out, opt-in and audit request forms, use a secured HTTPS connection only.

Although we use measures to help protect your information against unauthorized access, use or disclosure, we cannot guarantee the security of information provided over the Internet or stored in our databases. Your transmissions are, therefore, at your own risk.

Our Site may include links to third party sites (“Third Party Sites”), which are included to serve as a resource for our users. HealthInfoNet reserves the right to change this policy at any time.  We will post any such changes to this Privacy Policy, which changes will become effective after they have been posted.  By continuing to use the Site after having an opportunity to review the updated Privacy Policy, you have accepted the revised terms.  If you do not agree with such revised terms, please exit and do not use the Site.

7.  Governing Law.

This Privacy Policy will be governed by and construed in accordance with the laws of the State of Maine, without giving effect to its conflicts of law provisions. By accessing or using the Site, you consent and submit to the exclusive jurisdiction and venue of the state and federal courts located in Maine.  A printed version of this Privacy Policy will be admissible in judicial and administrative proceedings based upon or relating to this Privacy Policy to the same extent and subject to the same conditions as other business documents and records originally generated and maintained in printed form.

8.  Miscellaneous.

In the event that any provision of this Privacy Policy conflicts with applicable law, rule, regulation or order or if any provision is held invalid by a court with competent jurisdiction, then such provision will be deemed to be restated to reflect as nearly as possible the original intentions of the parties, and the remainder of this Privacy Policy will remain in full force and effect.
